Leaving the CCG
As a way of improving staff experience and retention, HR have developed an Exit Questionnaire and Exit Interview process.
The new Exit process has been developed with support from the Engaging our People Forum.
The Exit Questionnaire will be available for staff leaving the CCG. Leavers will be able to complete the questionnaire online through a secure platform called QuestionPro.
Staff can also opt to have an exit interview with either their line manager or HR. If staff would prefer HR to conduct their exit interview, they can contact HR at firstname.lastname@example.org.
The Exit Questionnaire in broken down into various themes which will allow the CCG to monitor staff experience during the time the member of staff has been with the CCG.
Managers are required to complete a Leaver Form within one week of receiving written confirmation of the staff member's leaving date. The Leaver Form is available in the downloads section on this page and includes:
- Information which will ensure the payroll and HR system is updated to reflect the staff member's leaving date
- A checklist for things you need to do with your staff member before they leave
- Guidance on how to complete the form
